This page will walk you through the required forms to be filled out (including financial forms toward the bottom), to move your ministry event forward.

The Process
1)
Event Planning Overview Form - due at least 4 weeks prior to event announcement
To begin the process, please fill out our Event Overview Form at least 4 weeks prior to when you want your event announced, or 8 weeks prior to the event (whichever is soonest). Once you complete this form, a copy will be emailed to you. Based on your entries, you may need to fill out additional forms below (i.e. Kitchen, A/V, etc.). Please reference this email to know which forms you will need to fill out in the future; you'll see a heading with "Please fill out the [Form Name]" with a link to the form.

Additional Forms - due at least 4 weeks prior to event
These forms inform various ministry leaders about what will be needed from them or their ministry. Below is a list of ALL the forms. Based on your answers to the above form (please reference the copy you received via email), you may not need to fill all of them out. If you cannot find your copy of the form, or are still unsure which you will need to fill out, begin filling out the Event Planning Followup Form (you can find this in Step 3 below), which, if a form is needed, will specifically ask if you filled out that form.

Event Planning Followup Form - due at least 4 weeks prior to event
Once you have filled out all the additional forms needed (or if you need to use it to determine which of the above forms need to be filled out), please fill out this final Event Planning Followup Form, to provide the church office with final details.
Financial Forms
If you need to request a reimbursement for someone who purchased something for your event, or request a check (say for a speaker, etc.), please fill out the appropriate form below (or provide the link to the person needing the reimbursement, as it will require the receipts). The appropriate ministry leader will be asked to approve the reimbursement before a check will be written.
